Zinc + FolateLow Sperm Count

Zinc + Folate for Sperm Count: The 74% Increase Explained

The zinc + folate combination produced the largest supplement-driven sperm count improvement in the fertility research literature.

The Wong et al. (2002) Fertility and Sterility trial randomised subfertile men to zinc sulfate, folic acid, zinc + folic acid, or placebo for 26 weeks. The zinc + folate combination produced a 74% increase in total normal sperm count — more than twice the improvement of either compound alone. This synergy is now one of the most cited findings in male fertility supplementation.

Why the Combination Works

Zinc and folate address complementary rate-limiting steps in spermatogenesis. Folate drives DNA synthesis (thymidylate synthase requires 5,10-methylene-THF to synthesise thymidine for DNA replication). Zinc stabilises the chromatin structure of newly formed sperm and supports the Sertoli cell environment where spermatogenesis occurs. When both are adequate, spermatogenesis runs at higher efficiency. When either is deficient, spermatogenesis is bottlenecked at a different point. The combination removes both bottlenecks simultaneously.

Active Forms Matter

The Wong trial used zinc sulfate (moderately absorbed) and folic acid (the synthetic form requiring MTHFR conversion). Updated protocols using zinc picolinate (highest bioavailability) and L-methylfolate (the active form that bypasses MTHFR) should theoretically outperform the original study in the 40% of men carrying MTHFR variants. The 30mg zinc picolinate + 800mcg L-methylfolate used in ApexFertility reflects this updated formulation approach.
