The Cortisol-Testosterone Antagonism

Cortisol and testosterone are physiological antagonists. Elevated cortisol suppresses GnRH pulsatility at the hypothalamus → reduces LH secretion → reduces Leydig cell testosterone output. This is an evolutionarily conserved mechanism: during perceived stress, the body down-regulates reproductive investment in favour of survival resources. Chronic psychological or physiological stress therefore chronically suppresses testosterone via this pathway.

Ambiye et al. (2013, Evidence-Based Complementary and Alternative Medicine) — 46 infertile men, 90-day RCT — found KSM-66 at 675mg/day produced a 17% increase in testosterone, 167% increase in sperm count, and 57% increase in sperm motility. Chandrasekhar et al. (2012) in 64 chronically stressed adults found 27.9% cortisol reduction with 600mg/day. Wankhede et al. (2015) found significant testosterone increases in resistance-trained men on KSM-66 vs. placebo after 8 weeks.

Who Benefits Most

The testosterone effects of ashwagandha are most significant in men with elevated cortisol or physiological stress. Men who train heavily (overtraining elevates cortisol), work demanding jobs, have poor sleep quality, or are in generally high-stress periods show the largest testosterone responses.
